About GLP-1 Access by State

What the site is

Fifty-one state pages, one structured data file behind them, six explainers, and a lookup tool. Each state page is a card with six rows: the medical board licence lookup, the board of pharmacy, Interstate Medical Licensure CompactIMLCAn agreement between states that speeds up licensing so a doctor can practise in more than one state. membership, the telehealth in-person exam rule for non-controlled drugs, the Medicaid fee-for-service position on GLP-1s for obesity, and compounding notes. Every row names its source and the date we checked it. The explainers cover the things that do not fit on a card: how to run a licence search and read the result, how telehealth prescribing works across state lines, what the compact is and is not, why Medicaid coverage of obesity drugs varies and changes, how to read a preferred drug list, and a glossary.

Who runs it

The site is written and maintained by the FormBlends editorial team, which also publishes formblends.com. FormBlends operates a GLP-1 weight-loss program that dispenses compounded medication through licensed pharmacies. That is a commercial interest, and it is why this site does not recommend clinics, prescribers or pharmacies, including FormBlends. It lists the public lookups and the public rules and leaves the choice to you.

How the data file is built

• Licence lookups. Each state medical board's lookup address was requested from a server on the date shown with a desktop browser user agent. A 200 response (or a 202 from a single-page application) is recorded as checked. A 403, 405, 503 or bot wall is recorded as browser-only: the address is still the board's published lookup, it just refuses automated visitors. Board of pharmacy links are imported from the Compounding Explained site's verified list rather than retyped.
• Compact membership. Read from the Commission's participating-states map at imlcc.com, with the map's own dates for states still implementing. The older imlcc.org domain no longer belongs to the Commission and is not linked anywhere on this site.
• Telehealth rules. A state is marked "no in-person exam required", "allowed with conditions" or "in-person exam required" only when we loaded the statute, administrative rule or board policy text and it answers the question for non-controlled drugs. The citation and a link to the text appear on the card. Where we have not read the text, the card says "check with the board". We do not copy rules from secondary summaries, however reputable.
• Medicaid. The baseline is KFF's state map in "Medicaid Coverage of and Spending on GLP-1s" (published January 16, 2026, data as of January 2026). Where a state Medicaid agency, its pharmacy benefit administrator or a plan bulletin says more, that source replaces KFF for the state, with its own date. "Covered with prior authorizationprior authorizationApproval an insurer requires before it will pay for a medicine. criteria" is used only when the state source states the criteria.
• Compounding notes. State-specific notes appear only where we loaded a state statute, rule or board notice. Every card carries the federal baseline from FDA's own pages.

Update cadence

The whole data file is reviewed quarterly: every lookup address is re-requested, the compact map and the KFF map are re-read, and each cited statute is re-opened. Between reviews, a reader report or a board notice we come across triggers a single-state update. Every page shows the date of its data, and each field carries its own check date where it differs.

Editorial rules

No reviews, testimonials, star ratings, named clinicians or "reviewed by" lines. No clinic, prescriber or pharmacy recommendations. Compounded drugs are described as what they are: not FDA approved and not interchangeable with brand products. A legal rule we have not read is not stated. Educational, not legal or medical advice.

Report a broken link or a changed rule

Email hello@formblends.com with the page URL, what changed, and the source (a board page, a statute, a Medicaid bulletin). Broken lookup links and changed rules are fixed as a priority and the date on the page moves. Reports of anything on this site that reads like a recommendation are equally welcome; that is not what it is for.
